Specific Details To improve Heart Failure care in Ireland, Develop eHealth Strategies to enable modern health care delivery for chronic illness

Intended results Prevention • To establish nationally HBT-developed and approved strategy • Potentially to “remain on the pitch” after successful national development to provide a “hub” role for advice to regional centers so as to ensure that ongoing further improvements of this strategy will be disseminated to all areas • To advocate for this development at wider international level, a task facilitated by the involvement of the HBT in international consortia looking at this strategy EHealth • To establish Virtual Consultation and other forms of eHealth as part of the bedrock of health care delivery in Ireland, as a means of providing speedier, more effective interaction between primary and secondary care.
